- Original Date of Birth certificate issued by the Municipal Committee/CMO or school leaving certificate of the last school attended (Approved)
- Result/Report card/Mark Sheet of the last exam/class passed.
- Pass port size photograph of the pupil (for the day scholar-3 and for boarding students-6) and also a pass port size photograph of each of relatives/visitors/guardian to the boarding student.
- Medical Certificate of fitness.
